VA PGHD FHIR R4 Governance Implementation Guide

Overview

This Implementation Guide defines the base governance profiles and constraints for VA Patient Generated Health Data (PGHD) in FHIR R4 format.

Purpose

The VA PGHD FHIR Governance provides centralized validation rules and constraints that ensure data quality and consistency across all PGHD client implementations.

Key Features

  • Centralized Governance: Define validation rules once, enforce everywhere
  • Observation Profiles: Base profiles for all PGHD observations
  • Patient Profiles: Standardized patient identity and tracking
  • Source Tracking: Required meta.tag constraints for data provenance
  • Status Validation: Enforce final status on observations
  • Code System Validation: Ensure proper terminology usage

Governance Constraints

All profiles extending from this IG automatically inherit three critical invariants:

  1. pghd-meta-tag-1: Requires source tracking via meta.tag with system https://wiki.mobilehealth.va.gov/x/Onc1C
  2. pghd-observation-status-1: Enforces observation status = final
  3. pghd-observation-codesystem-1: Validates code system usage

Usage

Client implementations should extend VAPghdObservationProfileBase to automatically inherit governance constraints.
